Southwest Key's award-winning Employee Wellness Program is designed to ensure all of our 1,300 employees can live a healthy, happy life. Note: Some wellness initiatives are limited to permanent full-time (benefit-eligible) employees.

Employees: Complete a minimum of 4 initiatives from each group. Each group completed will earn you $50. Successfully complete all 4 groups and earn an additional $100.
Groups can be completed in any order.
Submit forms along with supporting documents for each completed initiative to Attn: Wellness Coordinators to wellness@swkey.org, fax 512.912.7690, or mail 6002 Jain Lane Austin, TX 78721 following the Southwest Key Wellness Program Reimbursement Schedule. To protect your confidentiality some supporting documentation will be submitted to a third party, Gallagher Benefits Services, when specified.
